New Student? Get the Information Needed for Enrollment

                                      
If you are coming from another school district you will need to bring the student's:

• Birth Certificate

• Social Security Card

• Immunization Record

• Recent Physical Examination

• Achievement Test Scores

• Current Report Card

• Proof of Address (3 items)

• Parents Photo ID

If you are coming from another Camden Public School you will need to bring the student's:

• Transfer/Withdrawal Papers

• Current Grades and Test Scores

• Proof of Address

• Medical Records

To expedite the time needed to complete the enrollment process, print and complete any forms relevant to your child's transfer and bring them with you. These forms are also available in the Guidance Office.
